Twitter is stream of consciousness posting. You do something, meritorious, mundane, or malicious, and you post it. Your friends read it, and reply, or simply absorb. Followers indicate how many people actively review your content. Friends indicate how many persons' content you actively review. I have about 600-700 people that I watch now. Some of it is meaningless, but much of it is interesting. It's basically the newscrawl of your slice of the internet.
